QFM IS CURRENTLY CLOSED TO NEW PATIENTS.  WE ARE ACCEPTING FRIENDS AND FAMILY MEMBERS OF EXISTING PATIENTS.  PLEASE CALL THE OFFICE IF YOU KNOW SOMEONE WHO IS INTERESTED IN JOINING QFM.

Please keep your own copay receipts!   If you file the receipts you get at your office visit, then you'll have them for your year-end accounting needs.
