QFM IS CURRENTLY CLOSED TO NEW PATIENTS. INTERESTED PATIENTS MAY SIGN UP ON OUR WAITING LIST BY CALLING THE OFFICE.

We have ACH available finally! As a result of many of your suggestions, Qfm was able to configure an automatic billpay system where your copays and other fees can be automatically deducted from your checking account. Thank you for your patience as we transition to this program in the Fall.

Please keep your own copay receipts! As of March 1, 2008, Carolyn will no longer be providing yearly totals for each individual patient. If you file the receipts you get at your office visit, then you'll have them for your year-end accounting needs.

As of February 1, 2008, Qfm can not accept American Express. We will continue to be able to process VISA & Mastercard debit or credit cards. We apologize for the inconvenience.
